Marine Quest Diving Forms
Thanks for enrolling in Marine Quest! Since your camper
will be diving with us at Aquatic Safaris SCUBA Center, we
need you to provide us with some information and agree to a
few standard documents. There are two sets of documents,
those the for the dive shop and those for PADI. More
info on this below.

Please send your completed forms to us as quickly as possible after you are registered in the camp.
Most of the dive programs use PADI's eLearning system for
knowledge development. Therefore, we ask for the
camper's email address to be used in PADI's system and that
you provide this information to us at least 2 weeks prior to
camp start date. Also, included on PADI's portal
are electronic forms for the student diver. These are
PADI's GL Release forms and the student's Diver Medical |
Participant Questionnaire. The Medical form will
instruct if the student diver needs physical clearance before
participating in the class.
Please select your program and session below to download the correct dive shop forms packet. Read through the instructions, as well as the cover sheet on the forms packet before you start.
Instructions:
- Use Acrobat Reader, or equivalent, to fill out forms electronically. There are multiple digital signatures on the forms. Acrobat will prompt you to save the document each time it is signed. Simply "Cancel" the save action until all the signature fields are complete before saving the final document. Once complete, submit the signed forms to us through e-mail to mqinfo@aquaticsafaris.com.
- There are important program-specific instructions on the cover page of the PDF. Please take the time to read and understand what you're filling out and agreeing to.

A note about dates:
PADI uses the international DD/MM/YYYY format for dates on its forms - For example, Christmas will be on 25/12/2017. Most Americans are used to writing the opposite (12/25/2017). Please use the international DD/MM/YYYY format to avoid confusion. - Send forms in as early as possible.
If your program starts in less than a week:
In order to ensure that you can participate, please communicate with us and be sure that forms are complete, accurate, and legible. In addition to e-mailing electronic copies, bring the originals with you to check-in just in case. -
Important:
Determine if you need a physician's clearance to dive by completing the forms on PADI's portal.
Answer each question on the Medical Statement by writing "YES" or "NO" (please no blanks, checks, N's, X's, etc). Any "YES" answer to a question on the Medical Statement indicates a need to obtain a physician's clearance to dive. Please take the time to read paperwork and instructions
carefully, and make sure forms are complete and legible before
submitting.
